Some of the health plan expansions approved for 2025 cover only certain ZIP codes within a county. The exact service areas are still contingent upon successful provider negotiations, completion of the 2025 rate development process, as well as Department of Managed Health Care (DMHC) approval where applicable.

- Select the county, where applicable.
- Search the table for the ZIP code.
If the ZIP code appears in the table, then the respective health plan may expand there in 2025.
